# Harm OCD: the thoughts you would _never_ act on.

You're chopping vegetables and a picture flashes through your head. Or you're driving and suddenly wonder what would stop you from swerving. Or you're holding someone you love and a thought shows up that makes you want to put them down and walk away.

You would never. You know that. But "knowing" doesn't seem to count, because the question keeps coming back: what if I lost control? If this sounds familiar, it's worth knowing this pattern has a name, and a well-established way of treating it.

## You've built a whole life around _not_ being trusted with yourself.

- Knives, scissors or medication get hidden, or you just avoid being near them.
- You avoid driving, or you circle back to make sure you didn't hit someone.
- You avoid being alone with a baby, a pet, or someone you love, just in case.
- You scan your memory to confirm you didn't do something terrible without noticing.
- You check your feelings after a thought to see if some part of you liked it.
- You confess the thoughts to a partner so they can tell you you're not dangerous.
- News stories about violence stick to you for days.

How ERP works here

## We treat the _doubt_, not the thought.

Harm OCD runs on one impossible demand: be 100 percent sure you'll never hurt anyone. Every avoidance and every check is an attempt to get that certainty. And each one teaches your brain the danger was real, so the doubt gets stronger.

ERP works by gradually approaching the situations you've been avoiding while letting go of the checks and reassurance. Maybe that starts with leaving a knife on the counter while you're in the room. Later, using it to cook dinner with someone nearby. Later still, driving a familiar route without looping back.

None of this involves doing anything dangerous. You're not practicing harm. You're practicing living with a doubt you can't fully answer, which, if we're honest, is something every human does all day long.

We move at a pace you agree to. You're never tricked into an exposure. If a step is too big, we break it into smaller ones. CBT tools help alongside, especially for beliefs like "having the thought makes me a bad person."

And we don't spend sessions proving you're safe. That would just be more reassurance. We spend them building the skills that make the question matter less.
